Ca lâm sàng: Xử trí nhồi máu cơ tim cấp ở bệnh nhân có tiền sử ghép thận

Bệnh nhân nam giới 75 tuổi có tiền sử tăng huyết áp, đái tháo đường típ 2 và ghép thận nhập viện vì đau thắt ngực trái kèm khó thở giờ thứ 6. Bệnh nhân được chẩn đoán nhồi máu cơ tim cấp có đoạn ST chênh lên kèm suy tim cấp và được điều trị bằng lợi tiểu, chống đông, kháng kết tập tiểu cầu kép và chụp động mạch vành qua da. Kết quả chụp mạch vành tắc hoàn toàn động mạch liên thất trước đoạn 1 (LAD 1), bệnh nhân được can thiệp đặt 1 stent phủ thuốc trên đoạn tắc LAD 1. Bệnh nhân dần hồi phục và được xuất viện sau đó 2 ngày bằng các chiến lược điều trị kháng kết tập tiểu cầu kép, tối ưu hóa thể tích dịch và điều trị tình trạng suy tim. Báo cáo của chúng tôi sẽ thảo luận sâu hơn về chỉ định, liều dùng của các thuốc quan trọng trong bệnh động mạch vành như thuốc kháng kết tập tiểu cầu, statin, chẹn giao cảm và nhóm ức chế men chuyển trên đối tượng bệnh nhân ghép thận. Mặt khác, chúng tôi cũng đề cập tới lợi ích của can thiệp động mạch vành qua da ở những đối tượng bệnh nhân này.
