Viêm nội tâm mạc nhiễm khuẩn là một bệnh lý phức tạp, chẩn đoán tương đối khó khăn và đòi hỏi cần phải có sự kết hợp nhiều yếu tố bao gồm lâm sàng, hình ảnh học và vi sinh. Tiêu chuẩn Duke trong chẩn đoán viêm nội tâm mạch nhiễm khuẩn lần đầu tiên ra đời vào năm 1994, cùng với sự tiến bộ của các phương tiện chẩn đoán, sự thay đổi về đặc điểm dịch tể cũng như sự hiểu biết của chúng ta mà tiêu chuẩn này liên tục được cải tiến và chỉnh sửa nhằm mục tiêu nâng cao khả năng chẩn đoán để góp phần cải thiện chất lượng điều trị đối với viêm nội tâm mạc nhiễm trùng. Do đó Hiệp hội bệnh truyền nhiễm tim mạch quốc tế (ISCVID) đã xây dựng một hướng dẫn mới nhằm cải thiện chiến lược chẩn đoán bệnh nhân viêm nội tâm mạc nhiễm khuẩn. Trong bộ tiêu chuẩn mới này đã có nhiều thay đổi đáng kể bao gồm các xét nghiệm vi sinh mới, hình ảnh học cũng như những tiêu chuẩn quan sát trong phẫu thuật tim.
